    1. Did the Financial Consultant ask you relevant questions in order to understand your financial situation, investment objectives and risk appetite before recommending the financial product to you?
    2. Was there any instance during the financial advisory process where your Financial Consultant influenced you to modify your original answer to one that does not represent your intention and that you are uncomfortable with?
    3. Did you understand the Financial Consultant's explanation as to why the financial product is suitable for you?
    4. Did the Financial Consultant inform you of the following?
      For Insurance/Investment-Link Plans -
      You will not get back the full amount paid if you terminate the plan early;
      There are investment risk on the premiums paid for investment-linked insurance plans; and
      There is a 14-day free look period, where you may cancel the insurance or investment-linked insurance plan (as the case may be) within 14 days from the date on which you receive the policy from the insurance company, and if so, you would be returned your monies or premiums.

      For Unit Trusts -
      Investment in unit trusts is subject to investment risk; and
      There is a 7-day cancellation period and you would be returned your monies, including the sales charge.
